The documents below are part of arranging a funeral or cremation. You're welcome to download, read, and print them at home, or bring nothing at all, and we'll fill them out together.

Authorization for Release and Removal

Gives us permission to bring your loved one into our care from the place of death. This is usually the first form, signed by the closest living next of kin.

Death Certificate Information

The details we need to file the State of Illinois death certificate — full name, date of birth, family information, and more. Fill in what you know and we'll help with the rest.

Authorization for Cremation and Disposition

Used when cremation has been chosen. It authorizes the cremation and tells us how the cremated remains should be returned or placed.
